Acceptance:
1. The program coordinator and faculty review all
completed applications and records and render a joint
decision.
2. The applicants are notified, in writing of the
programs final decision.
3. Applicants return a self-addressed stamped postcard
confirming their decision to enter the practical nursing
program.
4. A dental exam is required by the State Board of Nursing.
The applicant assumes all costs and fees for the dental
examination.
5. The applicant must schedule a Physical examination with
their physician. The applicant assumes all costs and fees.
***The following immunizations and laboratory tests are
required for Program entry.
***Fee for urine drug screening will be included in costs
to the Program.
Immunizations:
A. Hepatitis B Vaccine and follow-up antibody titre.
B. MMR or proof of immunity to Measles, Mumps, and
Rubella. (measles, mumps, rubella titres)
C: Varicella history and/or Varicella titre.
D. Diptheria/Tetnus.
E. Tuberculin Screening---*Must be completed within 90
days of Program entry.(2-step)
Laboratory Tests:
A. CBC-Complete Blood Count
B. UA-Urinalysis
C. RPR-Serology
D. Urine Drug Screen for the following:

cannaboids
-
cocaine
-
opiates
-
phencyclidine
-
barbiturates
-
benzodiazepines
-
amphetamines
*Students are required to obtain a copy of laboratory
test results for there student file.*
***Any applicant who meets the standards for admission
is eligible for admission to the Practical Nursing
Program. Candidates are seated until the class is
filled to a maximum of 36 students per class.***
