Procedures:
1. Scheduled examination time and date for the NET
pre-entrance Examination with the practical nursing
program secretary. The applicant must pre-register for
the NET and pay the NET examination fee.
2. Complete and submit the application form, as
furnished by the practical nursing program.
3. Obtain official high school and/or college
transcripts or GED scores.
4. Obtain three personal references using the program
nursing reference form.
5. Schedule personal interview with program coordinator
or faculty through the practical nursing program
secretary.
6. Submit completed medical and dental forms prior to
first day of class.
7. Applicants must secure form SP 4-164 (Act 34
Clearance and Act 33 Clearance) from the program office.
Completed form to be returned to the practical nursing
program. Applicants are responsible for fees.
***An applicant will not be permitted to the NNCLEX exam
for licensure if he/she has been convicted of a
felonious act prohibited by the act of April 14, 1972,
(P.L2333 No. 64), known as "The controlled substance,
drug, device and cosmetic act", or convicted of felony
relating to a controlled substance in a court of law of
the United States or any other state, territory, or
country unless at least ten (10) years have elapsed from
the date of conviction. The term, "convicted" shall
include a judgment, an admission of guilt, or a plea of
nolo contentendere.***
